What is Home Health?

Home Health care focuses on restoring a patient’s health and independence in the comfort of their own home.

Reliable Home Health offers a broad menu of services, ranging from physical therapy to nursing visits and other rehabilitation services, all focusing on restoring a patient’s health and independence.


WHO QUALIFIES

According to Medicare, the following five eligibility requirements must be met:

1. You are receiving services under an established plan of care that is reviewed regularly by a physician.

2. A physician certifies that you need intermittent skilled nursing care (other than drawing blood), physical therapy, speech-language pathology services, and/or continued occupational therapy.

3. The home health agency caring for you is approved by Medicare.

4. A physician certifies that you are homebound.

5. As part of your certification of eligibility, a physician, or other health care professional that works with a physician, must document that they have had a face-to-face encounter with you within the required time frame and that the encounter was related to the reason you need home health care.

Our team works with your primary care physician to develop an individual plan that is centered around the priorities and wishes of the patients and their loved ones. Reliable Home Health team may include a nurse, home health aides, social workers, physical, occupational and/or speech therapists.

How Does Home Health
Get Paid For?

As long as the patient is homebound, Medicare and most Medicare Advantage Plans (like a HMO or PPO) typically cover 100% of services when there is a skilled health-care need following an acute episode such as hospitalization or change in health status. Income is not a factor when home health care services are provided due to a skilled need under Medicare.

At-home care services may include physician visits, nursing visits, physical, occupational and/or speech therapy, medical social services, care from home health aides, homemaker or basic assistance care, nutritional support, laboratory and X-ray imaging as well as pharmaceutical services.
