Our Hospice Care program is designed to turn the focus of care from being curative to comfort and quality for our patients and their families.

Choosing hospice allows a patient an opportunity to ensure that are they are in control of their healthcare decisions and chose how they want their illness treated.

Hospice is not about giving up, but rather about making more meaningful moments possible.

who qualifies?

Reliable Hospice services are available to:
• Anyone 18 years and older
• Diagnosed with a life-limiting illness
• Curative treatments are no longer the primary focus
• Increased symptoms such as weakness, shortness of breath, weight loss, pain, require assistance with most daily activities
• Have had multiple hospital/ER stays

Reliable Hospice interdisciplinary team includes:
• Physicians
• Nurses
• Social workers
• Chaplains who provide spiritual support
• Home health/bath aides
• Volunteers
• Bereavement support

Our nursing team is available 24/7 to ensure our patients needs and comfort are managed day and night.

how is hospice care covered?

Hospice is paid for by the Medicare Hospice Benefit, Medical (or MediCaid) and most private insurance plans. In most cases, medical supplies, medical equipment and medications that are deemed necessary by the physician for the treatment of their illness are provided at no cost to the patient or their family. Reliable Hospice interdisciplinary team includes physicians who provide medical oversight, nurses who provide comprehensive assessments and manage patients symptoms and medical needs, social workers who provide emotional support and resources to help ease the life struggles, chaplains who provide spiritual support, home health aides who provide assistance with activities of daily living, bereavement counselors and trained volunteers. Our nursing team is available 24/7 to ensure our patients needs and comfort are managed day and night.

Reliable Hospice offers a full bereavement program to support families through the emotional rollercoaster following the death of their loved one.

Our bereavement program offers communication either through mail, telephone or in-person, based on the needs of our bereaved. We celebrate the lives of our patient’s yearly during our Celebration of Life ceremony. This is a beautiful day to honor the patients we have served.
